What to Bring to Your First Appointment
Please bring the following items to your initial visit: a valid photo ID (driver’s license or passport), your insurance card, a list of all current medications including dosage and frequency, any relevant medical records or recent lab results from a previous provider, and a list of any questions or concerns you’d like to discuss with the doctor.

We accept a wide range of insurance plans, including Medicare, Blue Cross Blue Shield, Cigna, Wellpoint, United Healthcare, Aetna, and most Marketplace insurance plans. What to Expect at Your First Visit
Your first appointment with Dr. Ezenwabachili will be thorough. He will review your complete medical history, discuss your current health concerns and goals, perform a physical examination, and order any necessary lab work or screenings. Dr. Ezenwabachili takes the time to listen and answer your questions, so you can expect an unhurried, patient-centered experience.

Bissonnet Internal Medicine PA is located at 7737 Southwest Fwy, Houston, TX 77074 — conveniently situated near Memorial Hermann Southwest Hospital. Office hours are Monday through Friday, 8:30 AM to 4:00 PM. Same-day appointments are available for both new and existing patients.
